Liability Waiver

Acknowledgment and Assumption of Risk

I, the undersigned, understand that participation in Classical Connections activities involves certain inherent risks. These may include, but are not limited to, risks associated with physical activities, use of facilities, travel to and from events, and interactions with other children and adults.

I acknowledge that I am voluntarily participating in the Classical Connections co-op activities with knowledge of the dangers involved and agree to accept any and all risks of injury, illness (including communicable diseases), or damage to property.

Release of Liability

I understand that by participating in Classical Connections activities, I am accepting responsibility for the risks involved. I agree not to hold the co-op, its leaders, volunteers, or host facilities legally or financially responsible for any injury, accident, illness, or loss that may occur during co-op events or while on premises used by the co-op.

Indemnity Agreement

I further agree to indemnify and hold harmless Classical Connections, its leaders, volunteers, hosting facilities and affiliated entities from any and all claims, actions, damages, liabilities, and costs (including reasonable attorney’s fees) brought by any third party as a result of any action, inaction, or conduct of myself or my child(ren) during participation in co-op activities. This includes, but is not limited to, claims related to injury, damage to property, or violation of co-op rules or policies.

Medical Emergencies

In the event of an emergency, I authorize co-op leaders or volunteers to seek medical treatment for my child(ren). I understand that I am responsible for any medical expenses incurred.
